Recommendations are based on the information available at the time of consultation.
Appointments - Injury assessment and rehabilitation
Appointments are available by booking only.
Initial Assessment: £70 (60 minutes)
Exercise Rehabilitation Session: £70 (60 minutes)
Fees may be reviewed and updated periodically.
Payment - Injury assessment and rehabilitation
Payment is due at the time of booking unless otherwise agreed.
Cancellations - Injury assessment and rehabilitation
A minimum of 48 hours' notice is required for cancellations or appointment changes.
Appointments cancelled with less than 48 hours' notice may be charged at the full appointment fee.
Late Attendance - Injury assessment and rehabilitation
If you arrive late, your appointment may need to finish at the scheduled time to avoid affecting other clients.
Health Information
Clients are responsible for providing accurate and up-to-date information regarding their health and medical history.
Workplace Ergonomics Service
Workplace ergonomics service provision will be agreed by the client prior to service delivery. Once complete an invoice will be issued. Payment is due 30 days from date of invoice.
If cancelled within 5 working days of the agreed service delivery date a 100% charge will be incurred.
No Medical Emergency Service
Active Return does not provide emergency medical care.
If you require urgent medical assistance, contact NHS 999, your GP, or emergency services as appropriate.
Liability
While every reasonable effort is made to provide safe and evidence-informed services, no healthcare intervention can guarantee a specific outcome.
